ConnectCARE is a Personal Emergency Response Service owned and operated by Halton Healthcare Services, located inside Oakville Trafalgar Memorial Hospital. ConnectCARE allows immediate access to help from your home with the simple push of a button, 24-hours a day. How does ConnectCARE work?

CALL FOR HELP
When you need help, simply press the button worn on your neck or wrist. This will activate the ConnectCARE speaker unit attached to your home telephone line.

THE CONNECTION IS MADE
When our ConnectCARE monitoring staff receive your call for help they have immediate access to your vital information.

HELP IS ON THE WAY
Our ConnectCARE staff will speak with you to determine the nature of your emergency and find out what help you need (a family member, friend, or emergency services). Even if you are unable to speak with us, one of your contacts will be sent to ensure your wellbeing.

FOLLOW-UP
Our ConnectCARE staff will follow up to make sure that help has arrived.

A personal emergency response service is for people who:
- Have had a serious fall or are at high risk of falling.
- Are predisposed to serious injury in the event of a fall.
- Wish to remain active and independent despite their advancing years.
- Live with medical or physical challenges.
- Have recently been discharged from hospital.
- Are recently bereaved.
Help is available at the touch of a button, 24 hours a day, 365 days a year.
